You must connect this app with your health care provider. MyChart is a service your healthcare organization provides to give you access to your health record. Your records stay at the organization where you receive care.

You must either create an account or sign up using your existing account to your respective healthcare organization’s website. MyChart is powered by Epic, the software system used by the world’s best healthcare organizations. Once you are done setting up the account, you may now use the app to access your health information including your medications, test results, health summary, and immunizations, as well as access other services such as requesting prescription renewals and communicating with your care team.
